Rise Builds a Reputation


ACO Enablement Company Strides Into
the Changing Health-Care Landscape

Rise Health Inc., which rolled out new health-care technology in its Jacksonville, Fla., support center last summer to help physicians deliver accountable care solutions, has been nominated as "the boldest company" of 2011 in Northeast Florida.

The Rise Health entry is part of the "Innovate Northeast Florida" project to find, share and showcase bold people, projects, companies and nonprofits in the Jacksonville region. Its JAX Boldest contest celebrates innovation and creativity.

Rise Health is dedicated to reducing medical costs while increasing access to medical care, improving clinical quality and the overall patient experience. The company trained patient care coordinators to traverse state-of-the-art information systems that placed them as an extension of primary care practices. In 7 a.m. to 10 p.m. shifts, Rise Health staffers connected patients to primary care, often preventing unnecessary emergency room visits. The Jacksonville-based company launched a major project for a large Northeast hospital last year and is now making plans to provide accountable care solutions to health-care providers in Jacksonville and elsewhere.

"Our model antiquates payor models of care, disease and utilization management by moving evidence-based medicine to the front-end of the care planning process" says Rise President Gino Tenace.

Rise wants to help health care industry "survive and thrive as the industry transforms from fee-for-service to more accountable delivery models."  Company leaders are in Las Vegas this week at HIMMS12, an annual convention and exposition of health IT professionals.
